Wedding Facility Use — Integrity Church

Please be aware that this must first be approved by the Elders of Integrity Church and this form in no way guarantees use of the space in the ways and dates you have specified. The final agreement will be a contract between yourself and Integrity Church. This form only provides the information necessary to create a contract for use of the facilities. Integrity Church requires that you use our sound technicians if you plan on using the audio and lighting system. Integrity Church has a cleaning fee of $150 and you must let us know once your event is concluded so they can come and clean. Anything that you and your party have set up will need to be taken down and anything moved by you and your party must be put back. The pricing includes use of the facility for both the rehearsal and the ceremony. If you plan on having a reception there will be additional costs. If you are a member, there are some costs that will be discounted. We will let you know what that will be when we review the official contract. Please also note, Integrity Church will not allow alcohol for events due to liability. **If you agree, please type your full name as a means of a signature and we will begin a contract upon approval of the Elders at Integrity Church.
